ALL ON 4 TREATMENT

The whole process requires two visits to Turkey, 3 months apart. The first visit will be 7 days long with about 3-4 appointments. The second visit will be 10 days long and require 4-5 appointments.

Patients spend their arrival day getting familiar with the hotel mostly. On the second day we deliver you to the Dentares Smile Clinic with our VIP transfer service for a 30 minute appointment. You’ll complete a patient history and consent forms. Then our patient relations consultant will assist you with having an x-ray and bring you to the Head Physician, Dr. Öyküm for a live consultation.

During the consultation, Dr. Öyküm will be accompanied by our Maxillofacial surgeon, Dr. Dincer. They will discuss your x-ray results, 3D tomography, have an intraoral examination and then provide you with the healthiest treatment plan. If you have any special requests, questions or expectations, we will discuss this here. If the doctors agree that the all on 4 is the healthiest treatment plan for you, a pre-arranged surgery appointment is available if the patient is ready to move forward with their treatment.

The doctor uses computer software of the patients x-ray and tomography to find the best placement of implants. The patient relaxes in the dental chair and within sterile conditions. The surgeon and assistants sterilise their hands, wear gloves, masks, surgical gowns, hair nets and sterilise the patients mouths, inside and out. The treatment may begin. The maxillofacial surgeon applies local anaesthesia for a pain free operation. The gingiva (gum) is opened and bone exposed. The surgeon prepares the implant socket with the help of dental drills to the appropriate size and diameter. The implant is placed in the prepared socket with a little bit of bone graft. The gingiva is then closed with dissolvable stitches. The stitches disappear in 7 to 10 days.

Temporary dentures are then provided exact to the patients measurements by taking impressions on dental putty. Patients will ask, “will I have fıxed dentures?”. The answer to that question depends on the decision of the doctor. If the doctor finds that the bone structure during surgery is healthy enough to support the osseointegration of implants and the strain of daily bite pressures from a fixed denture. Then the doctor may assign the patient a fixed denture. Whether the denture is fixed or non-fixed, the patient is always advised to keep to soft foods for 3 months while the implants heal. If the patient is given non-fixed dentures, the doctor advises not to use them except for social aesthetic reasons for the first month. The bone structure is greatly reduced compared to healthy patients, so our first duty is to preserve what bone the patient has left.

All on 4 patients require 2 visits to Turkey. The first visit will be to extract remaining teeth, treat abscesses or other conditions and place implants. Patients can think of their 1st visit as the medical stage of treatments. 5 days in Turkey is enough to complete the extractions, implant surgery and provide a temporary denture. Patients will need to adhere to the doctors after care advice and take the prescribed medications: Painkiller, mouthwash, and a course of antibiotics. The patient is able to return home to heal for 3 months at a minimum. After 3 to 6 months, the patient has fully healed and can come back to Dentares for their second visit.

To create the most functional and aesthetic prosthesis, Dentures require 10 days total for their second visit. Patients will have a check up appointment where the doctor assesses a new panoramic x-ray and opens the implants to check up on the healing. Provided that all implants have successfully integrated with the bone structure, the patient can see the prosthodontist.

One of Dentares’ prosthodontists will take impressions of your jaws with dental putty. From these impressions, the doctor and lab technicians are able to build the prosthesis. Zirconia is the most preferred material for prosthesis due to the durability and strength to last over a decade with the correct oral hygiene habits.

The prosthesis is first created by building a basement material that connects all 4 implants together, perfectly distributing the compressive forces of biting. This is achieved through state of the art CAD/CAM computer technologies. A full arch prosthesis will contain 12 ‘crowns’ or prosthetic teeth. After the basement material is constructed, it’s a matter of building the zirconia crowns on top. By using the rehearsal method, we can make tiny adjustments to the full arches so that the bite comes together as smoothly as a puzzle would. Each tooth from the upper and lower jaw sits together in unison creating the most functional and most comfortable prosthesis. After that is achieved, the prosthodontist refines the front teeth to smile design parameters for the most beautiful smile.

There are a few things that can extend treatment time and affect the osseointegration (healing between bone and implant). Those are abscesses and after-care habits. Patients will discover from the x-ray & 3D tomography results in the consultation stage, if they have bone abscesses. These are little pockets (or cysts) of pus and bad bacteria. If the abscess (1 or more) is within the surgical area implants are to be applied. Then first, we must surgically extract the infection and place bone grafts. This can delay the treatment process a further 3 to 6 months depending on the size of the abscess and bone graft applied.

It’s not possible to speed up bone healing unfortunately, but we can provide good oral hygiene, quit smoking and eat a healthy, diverse diet to ensure the healing is not delayed or affected. Smoking cigarettes and vapes restricts vascular circulation to the gums and bone resulting in vital nutrients and antibodies not being able to reach the healing site. This can lead to an infection and a failed implant. Extending the treatment process by up to 3 to 6 months.

Alcohol causes oral dehydration, dilates the capillaries, and interrupts a process called osseointegration. Dehydration and dilation increases swelling and can lead to blood clots, leading to necrosis of the capillary. Affecting the transport of nutrients and healing factors to the implant area. Osseointegration is the process of bone to implant integration. The bone tissue grows into the microscopic pores and enhanced surface areas of the implant.

The temporary denture is created only for aesthetic reasons. The denture is made of a brittle acrylic and is prone to breaking with improper usage. Dentures should not be used functionally. This means that the denture is not made to withstand bite forces while chewing and is only meant for aesthetic reasons. For example you can talk to your work colleagues, friends and family with a confident smile. This is because dentures place force on the implants and can disturb healing. It will always be the priority of the doctor to ensure the maximum benefits of healing. This is to protect the patient. We need to preserve the bone structure we have.

For these reasons: smoking, drinking and inappropriate denture wear can disrupt the healing process and lead to complications; extending the process healing time to 3 or even 6 months.

The all on 4 treatment requires surgical risk and fully licensed and experienced surgeons to complete the treatment and so it can get pretty expensive. Comparatively, the all on 4 treatment can be a third of the price of western countries.

Long-term smokers and frequent drinkers may have a hard time for a few weeks or months looking after the implants post-treatment. It is recommended not to smoke or drink alcohol while the body recovers. Patients can use nicotine patches but it does not replace the habit of having the physical feeling of a cigarette in the mouth.

Some patients can struggle having no temporary dentures or non-fixed dentures because they need to take them out and clean them each day. They are only given as an aesthetic measure.

Having implants means that patients will need to wait 3 to 6 months. Patients will follow the hygiene advice of the doctor to ensure their implants heals and integrates with the bone in the best possible way. This means actively caring and attending to small changes, cleaning and so on.

Although rare, local complications that may occur during dental implant surgery are: nerve injury that may result in numbness; but is usually temporary and bleeding. Complications that may arise after surgery are: infection and failure of the implant to integrate with the bone.

In Turkey, the all on 4 treatment can be up to 70% cheaper than UK, USA and Western European prices. Of course it also comes down to the specific patient’s need. Some patients may have abscesses in their planned implant area so the infection(s) are required to be surgically removed and treated. They will need to wait 3-6 months for these areas to heal before implants can be placed.

The all on 4 treatment uses angled implants to provide stability in the back of the jaw due to the loss of bone. The all on 6 treatment is different because candidates for the all on 6, have sufficient bone structure to support 6 vertical implants evenly placed in the jaw.

The all on 6 treatment is considered the complete smile makeover because doctors have the potential to use all available treatments in combination with the all on 6 treatment to deliver renewed, happy smiles.

The all on 6 treatment utilises 6 implants evenly placed vertically to the bone structure on each jaw. This system is a great option for patients of all sorts of oral conditions. If the patient has a healthy amount of bone structure, the all on 6 is a great option to preserve bone and provide a permanent denture system.

In cases where the sinus cavity has invaded the bone structure of the upper jaw and there is still available bone. We can increase the amount of structure by lifting the sinus and placing bone graft. Implants in most cases are still able to be placed in combination with bone grafts and sinus lift operations. However this would extend the recovery time for 3 months to 6 months to allow sufficient healing. Sinus lifting and bone grafts are not always required, so long as there is a healthy amount of bone structure for the implants to be successfully placed and integrated.

On the lower jaw, unless there are divets, extraction sockets or abscesses in the bone structure, placing bone graft would not be necessary.

The preservation of bone structure is key to having a healthy set of teeth or dentures for the rest of our lives.

Any patient who will have full extractions or is already without teeth, and looking for a permanent denture solution, that has healthy bone structure. These patients are seeking to improve their quality of life, confidence and social engagement. Sometimes a hearty steak is not possible to eat by a person losing their teeth or experiencing pain when chewing hard foods.

Patients that present with bone loss on both jaws but have enough structure to support implant treatment. Also those with multiple missing teeth, cracked, broken or ground down teeth are suitable for the all on 6 treatment.

In most cases where the sinus cavity has invaded the bone structure but there is still more than a centimetre of bone structure, sinus lifting and bone grafts are an added treatment but still allow the candidacy of these patients for the all on 6 treatment.

In the lower jaw, the doctor needs to see enough bone structure between the gingiva (gum) and the nerve canal on the lower side of the jaw. If there is enough structure, then the doctor can place implants. If there is not enough structure, this could compromise the nerve canal and lead to numbness. So the doctor always plans the precise measurements of the implant and the true placement within the jaw. If there is not enough bone structure, then the all on 4 treatment may be recommended instead.

Including 6 implants per jaw, there are bone grafts, sinus lifting, extraction, abscess clean, membrane, and masseter botox injection. Not all of these treatments may be required.

Bone graft is a material that allows for the natural reabsorption to a patients own bone increasing the available bone structure for successful implant fusion.

Sinus lifting is a delicate procedure that involves surgical opening the bone structure and care fully lifting the sinus membrane and placing bon graft material for future successful implant osseointegration.

Extraction includes the removal of any tooth or damaged tooth that has now become a bundle of nerves (often yellowish-brown in colour)

Abscess cleaning refers to a build up of pus and bad bacteria within a pocket of the bone structure, often going undetected without the aid of an x-ray or tomography scan.

A barrier membrane is used in some bone graft treatments to keep the bone graft in place and the membrane keeps soft tissue and bone tissue separate. The membrane is often dissolvable and will be absorbed into the body over a month post-treatment.

The masseter is a muscle in the jaw responsible for the condition known as bruxism. This is a grinding condition brought on by stress and with a harmless 5 minute botox procedure, can be controlled.

It varies very from patient to patient. The patient gets to choose their preferred implant brand from Megagen®, Global D®, and Straumann®. They may also require additional treatments such as bone grafts, sinus lifting, extraction, abscess clean, membrane, and masseter botox injection.
